Tri-County Health Network, incorporated in 2010, is a nonprofit entity committed to improving the quality and coordination of health and healthcare services in the Dolores, San Miguel and Uncompahgre river basin regions of Colorado.
While the Network does not provide direct medical services, we have been instrumental in operating several Community Outreach Programs that address the needs of our communities by making health care accessible, affordable and available.
Through these programs we have enrolled 1,200+ individuals into Medicaid or CHP+; provided oral preventive services to over 1,000 children each semester; transported 600+ patients to/from their medical appointments; screened 400+ individuals for risk of diabetes and heart disease; and, provided outreach to 2,250+ patients of our network clinics that are diagnosed or at risk for diabetes or heart disease.
