Striving To Keep Everyone In Their Home For the Highest Quality of Life

TCHNetwork believes all people should be able to remain living as independently as possible in their home of choice. This can be a challenge for community members aged 60 and over and/or those living with a chronic condition or a disability. Our goal is to empower these individuals by educating them on support services available to them and by helping to coordinate and streamline access to these programs on their behalf.

The In-Home Assessment Process

We provide resource counseling when it’s challenging for someone to safely live in their home. Clients may need medical equipment, home health, or hospice services. They may be struggling with a chronic illness or have had an accident that is making it difficult to be mobile. Or they may simply be a caregiver who needs a break.

An individual, provider, or family member may request an in-home assessment for any of the services listed below. The client is informed about what their choices are for services, who the home care agencies are to pick from, who provides transportation, how to sign up for home-delivered meals, and more. If the client needs additional services, they are referred to Region 10 for assistance funding the homemaker and personal care services.  

For many people, an in-home assessment can be an intimidating idea. While people often get worried that an in-home assessment will lead to their referral to move to assisted living or a nursing home, the goal and purpose of the in-home assessment are only to get a good sense of what the client needs and how we can best connect them to supports so that they can remain safely at home for as long as possible.

After an assessment is conducted, a TCHNetwork Options and Benefits Counselor designs a unique care plan for that client. Components of this plan are wide-ranging and can include any of the following services, including but not limited to the following:

  • Transportation
  • Nutrition Programs
  • Family Caregiver Support
  • Ombudsman, Legal Assistance
  • In-Home Services
  • Information, Education and Assistance/ADRC
  • Health Promotion and Disease Prevention
  • Advocacy and Coordination
  • Medicare Counseling (SHIP)
  • Material Aid
  • Options Counseling
  • Volunteers who can provide in-home support or respite

San Miguel Palliative Support Services

TCHNetwork’s Palliative Support Services program works to enhance a person’s quality of life when they’re living with an acute or chronic condition by connecting members of the San Miguel County community with locally available resources.

After an in-home assessment or a client referral, TCHNetwork works to connect members of the community with:

  • Volunteers to pick up groceries, make pharmacy trips, assist with light chores or companionship calls
  • Durable medical equipment such as wheelchairs, walkers, and crutches free of charge
  • Education and outreach including support groups, end of life planning (i.e. durable power of attorney), and dementia caregiver support
  • Other available resources

Medicaid Care Coordination

Our Medicaid Care Coordination team works individually with Medicaid clients to help them navigate resources available to them and make arrangements for the care they need.

Wrap Around Care Coordination

Our Wrap Around care coordination service is currently offered in the West End of Montrose County to community members with complex needs who are over 18, and not on Medicaid. Wrap Around is a team-based planning process that builds on the strengths of a client or family to create a unique plan of interventions & supports.
