Our Care Coordination team is dedicated to supporting community members in navigating the often complex healthcare system. By taking the time to understand each individual’s unique needs, challenges, and goals, our care coordinators develop personalized care plans that address social, emotional, and medical needs. As our population ages and individuals face disabilities, our Aging & Disabilities program steps in to provide crucial support. From in-home assessments to benefits counseling and long-term planning, we empower older adults, individuals with disabilities, and their caregivers to navigate healthcare and community resources with confidence. By promoting independence and dignity, we help individuals live life on their own terms.

Despite years of wearing her Personal Emergency Response System (PERS) without incident, Kathy faced a challenging moment in 2023 when she fell in her bathroom and found herself unable to get up. Thanks to her PERS which she received through Aging and Disabilities (ADRC) supports, Kathy was able to swiftly summon emergency assistance, highlighting the critical role of technology in ensuring her well-being. Reflecting on her experience, Kathy shares,

“My PERS device from ADRC has truly been a lifesaver. It’s not something you think about needing until you do, and I’m grateful it was there when I had my accident. With the assistance of Tri-County Health Network’s care coordination, I’m exploring additional in-home supports to help me maintain my independence and safety. Their guidance has been invaluable in navigating the challenges of finding suitable care options in my area.”

Palliative Support: Providing Comfort and Compassion 

In times of illness or end-of-life care, our Palliative Support program offers compassionate care and support to individuals and their families. Whether it’s managing symptoms, providing respite for caregivers, or offering companionship, our team is there to ensure that individuals receive the comfort and dignity they deserve. Through our palliative care services, we strive to enhance quality of life and provide support throughout life’s journey. 

Our team can assess and connect community members to volunteer help and resources as well as loan out durable medical equipment to help you remain as independent as possible. If you or someone you know needs assistance with groceries, errands, home care, companionship, or a break from caregiver duties contact our team.
