Eligibility:
- Must live in San Miguel County
- Experiencing serious illness, managing chronic conditions, or in need of end-of-life or comfort care
- No age requirement
TCHNetwork’s Palliative Support Program provides compassionate, person-centered care for individuals living with chronic or serious illnesses in San Miguel County. This program offers comfort-focused support to improve the quality of life and help individuals remain safely at home.
Services include:
- In-home visits and emotional support
- Assistance navigating medical care and advance care planning
- Coordination with healthcare providers and community partners
- Help addressing social determinants of health (housing, food, transportation, etc.)
