High Fidelity Wraparound (HFW)

Support for Youth & Families Facing Complex Challenges

Eligibility: Youth and families who live or work in Ouray or San Miguel County

High Fidelity Wraparound (HFW) is an intensive, team-based planning process designed to support youth with complex emotional, behavioral, and social needs. Offered by Tri-County Health Network, HFW empowers families to lead the development of solutions that support their goals, especially when multiple systems (such as schools, behavioral health, child welfare, or juvenile justice) are involved.

The process brings together a supportive team comprising the youth, family members, natural supports (such as coaches, neighbors, and extended family), and professionals. Together, they develop and implement a highly individualized care plan that builds on the youth’s strengths and addresses identified needs across all areas of life.

Who HFW Helps

  • Families feeling overwhelmed by navigating services across multiple systems
  • Youth at risk of out-of-home placement due to behavioral, mental health, or safety concerns
  • Youth involved with multiple agencies (e.g., courts, DHS, schools, mental health, or juvenile justice)
  • Families who want a voice in how care is coordinated and who helps provide it

What We Provide

  • A Facilitator to coordinate the team and lead the planning process
  • A Family Support Partner who has lived experience navigating similar systems and provides peer support
  • A consistent team approach where everyone works together toward shared goals
  • Support in reducing risk factors, strengthening family voice, and achieving long-term stability

The 10 Guiding Principles of High-Fidelity Wraparound

  1. Family Voice and Choice
    Families and youth are at the center of the planning process and make decisions about their care.
  2. Team-Based
    The care team is built with the youth and family and includes natural and professional supports.
  3. Natural Supports
    The process prioritizes involvement from people the youth and family trust, like friends, neighbors, or mentors.
  4. Collaboration
    Everyone on the team works together to create and carry out one plan that meets the youth’s and family’s goals.
  5. Community-Based
    Services and supports are provided in the least restrictive, most accessible settings possible.
  6. Culturally Competent
    Services and planning are respectful of and responsive to the family’s culture, beliefs, and language.
  7. Individualized
    The plan is tailored to the youth’s and family’s unique strengths, needs, and circumstances.
  8. Strengths-Based
    The process identifies and builds on the strengths of the youth, family, and community.
  9. Unconditional
    The team commits to stick with the youth and family, even when things get difficult.
  10. Outcome-Based
    Progress is measured and regularly reviewed. The plan is adjusted to ensure goals are met.
