TCHNetwork conducted a Community Health Needs Assessment in 2021, a regional survey designed to better understand the health-related needs of people who live or work in San Miguel, Ouray, the West End, and Delta counties. (While we did not specifically target “east end” of Montrose County residents, we did receive some responses from Montrose and Olathe community members and have included them in the visualization.)
