Tri-County Health Network (TCHNetwork) continues to make a meaningful difference in the mental well-being of individuals and families across the region by providing compassionate support, care coordination, and trusted partnerships that connect people to vital mental health resources. We are committed to collaborating with our communities to improve health for everyone.

The following stories highlight the diverse ways our team supports clients in strengthening their mental and emotional well-being, increasing access to behavioral health services, and fostering long-term self-sufficiency. TCHNetwork’s vision is to build vibrant and healthy communities where everyone has the opportunity and ability to thrive.

Fostering Client Autonomy and Advocacy
Clients facing housing insecurity, limited public benefits, and chronic stress often carry a heavy mental health burden. One woman secured affordable housing after months of self-advocacy, encouraged by her TCHNetwork Care Coordinator. Another client, overwhelmed by confusing program rules and digital barriers, gained the skills and confidence to manage her benefits independently with ongoing support. These stories reflect that TCHNetwork believes health is more than healthcare.

Bridging Mental Health Gaps
Many of our clients have experienced significant emotional challenges—including grief, depression, and isolation. Our team has helped them connect to trusted counseling, behavioral health providers, and substance use resources. One older adult, who had initially resisted therapy, is now regularly meeting with a counselor and rediscovering joy in his life. Another client, struggling under the weight of family obligations, began prioritizing her mental health and initiated services for her household as well. These examples show that TCHNetwork exists to help clients thrive.

Supporting Newcomers and Families
When Lucia* (*name changed to protect client privacy) arrived in Southwest Colorado as a recent immigrant, she faced not only language and cultural barriers but also uncertainty about how to access healthcare. With the help of a Tri-County Health Network Enrollment Navigator, she learned how to enroll in Medicaid and was guided step-by-step through the system. For the first time since arriving, she felt supported, seen, and understood.

Meanwhile, a young couple expecting their first child needed help applying for Medicaid and SNAP. Overwhelmed and unsure where to start, they turned to TCHNetwork for guidance. Their Care Coordinator walked them through the applications, answered their questions in their native language, and followed up to ensure everything was processed. That support eased their stress and gave them the peace of mind to focus on their growing family.
