Supporting Whole Health Through Collaboration and Compassion

In rural communities, accessing comprehensive mental and physical health care can be challenging, especially for older adults with complex needs. One recent example from our Behavioral Health Care Coordination (BHCC) team illustrates how collaborative, person-centered support can make all the difference.

A community member over the age of 60, already receiving in-home services through our ADRC Options Counseling program, was referred to BHCC after experiencing intense grief and suicidal ideation following a significant personal loss. Recognizing the urgency and complexity of her situation, our BHCC team partnered closely with a compassionate local medical doctor and their practice to wrap supportive services around her.

Together, the physician, the Options Counselor, and the BHCC staff created a care plan that prioritizes both her mental and physical well-being. The member identified the need to speak with her provider about her recent loss and was supported in accessing the Behavioral Health Fund to cover those therapy sessions.

Through ongoing check-ins, grief support, and help navigating her options, the member is now working toward both short- and long-term goals. This collective approach, rooted in person-centered care and strong community partnerships, is helping her rebuild hope and stability in her health journey.
