Program Overview:
Tri-County Health Network’s Behavioral Health Navigator (BHN) program offers compassionate, personalized support to individuals navigating their behavioral health needs. Whether someone is facing anxiety, depression, trauma, or substance use challenges, our BHCC team helps connect them to services and walk alongside them through the process.
The Navigator assists clients in overcoming barriers to accessing or receiving services needed to maintain well-being and independence through direct services or referrals to other community resources. The BHN also provides cross-agency and system coordination, partnering with regional behavioral health resources to enhance collaboration.
Behavioral Health Support Services:
- Provide individualized care coordination to ensure timely access to therapy, psychiatry, and behavioral health supports.
- Screen for Social Determinants of Health (SDoH) needs and connect clients with resources such as food, housing, transportation, or financial assistance.
Support with applications and referrals to mental health scholarship funds, including:
- Behavioral Health Fund (BHF), Youth Behavioral Health Fund (YBHF)
- Recovery Access Fund (RAF)
Person-Centered Support Planning:
- Complete initial assessments to identify short- and long-term goals.
- Create customized action plans that reflect client priorities.
- Empower individuals to make informed decisions about therapy options, insurance, and community-based supports.
Resource Navigation for Long-Term Sustainability:
- Assist with securing sustainable health coverage (e.g., Medicaid, OmniSalud, Marketplace plans).
- Match clients with behavioral health providers who fit their financial situation and preferences.
- Ensure continuity of care even after funding or short-term supports end.
Community Outreach & Education:
- Raise awareness about local behavioral health resources.
- Partner with community organizations to promote wellness and reduce stigma.
- Facilitate collaboration between clients and providers to build long-term support networks.
Note: The BHCC does not provide life coaching, mental health, behavioral health counseling or therapy, or any clinical service, but can help connect people to these services.
