Empowering Our Aging Communities Through A Whole Person Approach: Tri-County Health Network’s Aging and Disabilities, and Palliative Care Resources

At Tri-County Health Network, we understand that health encompasses more than just medical care – it’s about addressing all the factors that impact a person’s well-being. With this philosophy in mind, we take a whole-person approach to addressing social determinants of health, aging and disability, palliative care and support for caregivers, behavioral health, physical well-being, and clinical care.

From Telluride to Nucla and Naturita, Norwood to Ridgway and Ouray, through our programs and services, we aim to empower individuals living in San Miguel, Ouray, and the West End of Montrose Counties to lead healthier, more fulfilling lives.
