Do You Or Your Family Need Health Insurance? Our Health Care Guides are here to Help!

Finding affordable insurance is a front-and-center issue for individuals and families across America. This can be especially difficult for those who reside in rural communities on the Western Slope.

Our goal is to help you navigate the complex and ever-evolving world of health insurance and enroll you or re-enroll you in health coverage. TCHNetwork’s team of Health Coverage Guides (HCGs) are knowledgeable about low-cost and no-cost options from Health First Colorado (Medicaid), Children’s Health Plan Plus (CHP+), and Connect for Health Colorado, the official health insurance marketplace for Coloradoans.

Medicaid/CHP+ Public Health Emergency Is Ending-Take Action!

  1. Update your contact info so you get info about your health coverage from Health First Colorado. If you have Health First Colorado (Colorado’s Medicaid program) or Child Health Plan Plus (CHP+) and you’ve moved within the last 3 years, make sure you can get important information about your health coverage. You have to respond to letters to see if you still qualify for health coverage. Those letters go to your address or email, so it is important that your contact information is up to date. Updating your address, phone number, and email is quick and easy. We can help update your information by emailing us at enrollment@tchnetwork.org, or by calling Tri-County Health Network at 970-708-7096 for help or to make an appointment.
  2. Renewal Process. Colorado is returning to normal renewal processes for Health First Colorado (Colorado’s Medicaid program) and Child Health Plan Plus (CHP+). Check your email, mail, and PEAK inbox and take action when you get official messages. You can see your renewal date in PEAK at CO.gov/PEAK at any time. If it’s time to take action on your renewal we can help!
  3. Take action. If you no longer qualify for Health First Colorado (Colorado’s Medicaid program) and/or Child Health Plan Plus (CHP+) we can help you shop plans and costs and find out how much financial help you may qualify for. This must be done within 60 days of your renewal date!

Email us at or call Tri-County Health Network at 970-708-7096 to schedule an appointment.

HCGs complete 40+ hours of annual training so they can help people easily compare plans, apply for financial help (as applicable), and purchase insurance. A considerable advantage of purchasing health insurance through the Connect for Health Colorado marketplace is that it’s the only place where you can qualify for financial subsidies on monthly premiums and tax credits based on your income level. Extra savings may also apply in the form of cost-share reductions for deductibles and copayments, so please be sure to ask an HCG about these opportunities.

If you’ve lost coverage through Health First Colorado, apply through Connect for Health Colorado within 60 days of losing your coverage to take advantage of the monthly and out-of-pocket savings on your healthcare.

Please review the chart below to see what savings you might be eligible for through the marketplace.
