Eligibility:

  • Live in Montrose, Ouray or San Miguel Counties
  • Be enrolled in Health First Colorado.

Need help navigating your health care and support services? We’re here for you.

Our Medicaid Care Coordination team helps people enrolled in Health First Colorado (Medicaid) get the care and resources they need, when and where they need them. We work with you to make health care less overwhelming and more connected to your daily life.

What We Do

  • Understand Your Needs
    We’ll listen to your health and life challenges, physical, mental, or social, and help create a plan that works for you.
  • Connect You to Services
    From finding doctors or specialists to getting help with food, housing, transportation, or mental health, we help link you to what matters.
  • Keep Everyone on the Same Page
    We make sure your care team (like doctors, therapists, and case workers) are working together, so you don’t have to repeat yourself over and over.
  • Support During Big Changes
    Need help after a hospital stay or switching doctors? We’ll guide you through those changes and make sure nothing falls through the cracks.
  • Help Manage Chronic Illnesses
    If you live with conditions like diabetes, asthma, or depression, we’ll help you stay on top of your care to feel your best.
  • Break Down Barriers
    No ride to your appointment? Trouble understanding your benefits? We’ll help problem-solving and find solutions.
