This form can be used to check the status of an applicant’s scholarship amount, effective dates, copay, and remaining sessions. If you don’t know the Client ID, it can be found on the applicant’s SMBHF approval letter.
Please note that this information is updated twice a month based on billing information from providers. Sessions that occurred after the most recent update, and any sessions that have not yet been billed will not be reflected in the data.
