ABOUT US
 
Tri-County Health Network (TCHNetwork) strives to enhance the overall health of our communities by identifying, developing, and implementing successful community programs. These initiatives focus on improving quality and reducing costs to increase access to care for our region’s residents. As a result, TCHNetwork has made remarkable progress in ensuring healthcare is accessible, affordable, and available to populations in our rural counties.
 
Our diverse range of programs includes food resource assistance, health insurance guidance and enrollment assistance, mental health therapy and educational courses, multicultural advocacy, care coordination, and much more. Each of these initiatives is designed to improve the well-being of our communities and contribute to a healthier, more equitable future for all.

OUR STORY
 
The Telluride Foundation, in collaboration with regional healthcare leaders, recognized the need to enhance local healthcare in San Miguel, Montrose, and Ouray Counties by integrating services. Through a series of discussions in 2008, Tri-County Health Network was formed, focusing on improving quality, coordination, and access to healthcare at lower costs.

Tri-County Health Network was incorporated in 2010 as a 501(c)3 supporting organization of the Telluride Foundation.
