What Is Patient Navigation?
Patient Navigation is a process by which a trained individual guides patients through and around their barriers to accessing the health care system.
Patient Health Navigator Program
Our Chronic Disease Outreach and Prevention program places non-clinical Patient Health Navigators (PHNs) in our four Network clinics – Basin Clinic, Uncompahgre Medical Center, Telluride Medical Center and River Valley Family Health Center. Our goal is to improve the overall health of patients with diabetes and cardio vascular disease.
PHNs offer patients education about the disease and engage them in lifestyle changes based on the guidance of their primary care physician. PHNs ensure patients follow up for routine visits and testing to help keep their chronic disease in check.
