Providing Community Members The Support They Need To Live Their Entire Lives In Their Own Homes

Imagine having a partner with dementia and being able to access respite care. Imagine healing from a heart attack and knowing a volunteer will bring you the food you need from the grocery store. Imagine fighting cancer and knowing that a team of volunteers is here to help you with household chores. Together – we can make this a reality! 

The goal of the Palliative Support Services program is to provide community members with the support they need to live their entire lives in their own homes. We focus on connecting clients with the resources that they need, with an emphasis on comfort, compassion, and dignity.

Program At a Glance

TCHNetwork’s Palliative Support Services program works to enhance a person’s quality of life when they’re living with an acute or chronic condition by connecting members of the San Miguel County community with locally available resources.

We provide resource counseling when it’s hard for someone to safely live in their home. Clients may need medical equipment, home health, or hospice services. They may be struggling with a chronic illness or have had an accident that is making it difficult to be mobile. Or they may simply be a caregiver who needs a break.

TCHNetwork works to connect members of the community with:

  • Home health nurses
  • Volunteers to pick up groceries, make pharmacy trips or assist with light chores
  • Durable medical equipment such as wheelchairs, walkers, and crutches free of charge
  • Education and outreach including support groups, end of life planning (i.e. durable power of attorney), and dementia caregiver support
  • Other available resources
