How we can help

Our team of Health Coverage Guides (HCGs) are trained to help individuals navigate Colorado’s state insurance marketplace, Connect for Health Colorado and assist in the enrollment process. HCGs can also assist with Medicaid/CHP+ and SNAP (below). We have Certified HCGs serving the communities of Ridgway, Ouray, Telluride Area, Norwood and the Westend including: Nucla, Naturita & Paradox. We can schedule individual appointments or speak to your employees.

Medicaid / CHP+ Assistance and SNAP

We are a Certified Application Assistance Site. Our Enrollment Navigators (eNavs) process and enroll eligible children, pregnant women, and families efficiently and effectively into entitlement programs such as Medicaid / CHP+ and SNAP (Supplemental Nutrition Assistance Program).

An eNav will guide you through the entire enrollment process – from assuring the enrollment package is complete and submitting the application; through approval and renewal. We have enrolled more than 1,200 individuals into Medicaid or CHP+ and have eNavs serving the communities of Ouray, San Miguel, and Montrose counties.
